Open only to United States residents. Contest entrants must be 18 years of age or older. Winners must show a valid photo I.D. to claim their prize. Employees of MonsterMedia LLC, DBA KLJZ-FM and KCYK-AM, participating retailers, their affiliates, subsidiaries, advertising and promotion, electronic or print media, and their immediate family members and/or those living in the same household of each are not eligible. Sponsors are not responsible for faulty or mistranslated telephone communications, or for entries that may be lost, delayed or misdirected, or for printing errors in the entry form or Official Rules. Void where prohibited by law. Decisions of judges are final. Limit one prize per family/household per 30 day period. No substitutions or transfer of prize permitted. All federal, state and local taxes are sole responsibility of winner. All federal, state and local laws apply. Potential winner must execute any required Affidavit of Eligibility/Release of Liability, Prize Acceptance Form within 5 days of attempted notification. Noncompliance within this time period may result in disqualification and an alternate may be selected. All material submitted becomes the sole property of the sponsor and none will be returned. Acceptance of prize constitutes permission to the sponsor and its agencies to use winner's name and/or likeness for purposes of advertising, promotion and public relations efforts and winner grants to MonsterMedia LLC, any and all rights to said use without additional compensation. Winner agrees that MonsterMedia LLC, their advertising, promotion, distribution and judging agencies, affiliates and employees shall have no liability in connection with acceptance or use of the prizes awarded herein and acknowledge that said parties have neither made nor are in any manner responsible or liable for any warranty, representation or guarantee, expressed or implied, in fact or in law, relative to any prize, including, but not limited to, its quality, mechanical condition or fitness.
